The following is the Gas Fitting Permit Application for Ayer, MA. Listed below are additional forms that may or may not be required in conjunction with the Gas Fitting Permit.

If you have any questions regarding submital of the permit, please email Samantha at building@ayer.ma.us.
All other inquires please contact David Theide, Gas Inspector at 978-846-4513.
After you have completed the form, you will need to upload the required documents.
  • Workman’s Comp. and General Liability Insurance
Permits will not be processed until we have received all necessary documents and payments. Please submit separate checks for electrical and plumbing permits.
