-- Any pages that require Notarization (i.e. SAVE Public Benefits Affidavit or Private Employer Affidavit) MUST be printed off and either submitted in person or by mail.

Any incomplete submissions received will NOT be processed.

-- If you need to reprint an Occupational Tax Certificate or are moving to another address (within the city limits), please notify the Occupational Tax Office.
