GREEN AND GOLD NOMINATION FORM

The Green and Gold Award recognizes youth and adults that have demonstrated a consistent effort of volunteer services to their club, county, project and/or the association. Nominations of adult leaders/volunteers must be certified leaders within the Jefferson County 4-H Program to be eligible for this award.
