You must provide forms of identification as proof of employment eligibility. You will not be scheduled for work before your forms and IDs are submitted. 

You must choose one Identification from List A or two Identifications from List B and List C. Upload the FRONT and BACK of each ID. Make sure the photos are clear and eligible. Please refer to the chart below for acceptable Identification.
