Instructions:

Please complete the following registration form completely and attach copies of any required licenses.

RESIDENTIAL BUILDING – Attach copies of the Residential Building Contractor’s License (both company and qualifying officer) and the Driver’s License of the person who holds said licenses.

COMMERCIAL BUILDING – It is not necessary to attach any licenses to register as a commercial contractor for commercial projects. You are not required to complete the bottom two (2) sections of this form.

ELECTRICAL/FIRE ALARM/SIGN SPECIALITY CONTRACTOR – Attach copies of Electrical Contractor’s and Master Electrician’s Licenses and the Driver’s License(s) for the person(s) holding said licenses.

MECHANICAL – Attach copies of the Mechanical Contractor’s License and the Driver’s License of the person who holds the Mechanical License

PLUMBING – Attach copies of Plumbing Contractor’s and Master Plumber’s Licenses and the Driver’s License(s) for the person(s) holding said licenses.

FENCE – It is not necessary to attach any licenses to register as a fence contractor.

LOW VOLTAGE – It is not necessary to attach any licenses to register as a low voltage contractor.
