After you have completed the form, you will need to upload the required documents.
  • Workman’s Comp. and General Liability Insurance
  • Manual J
  • Plumbing License.

Permits will not be processed until we have received all necessary documents and payments. Please submit separate checks for electrical and plumbing permits.
